SSS ID Application Made Easy
If you have been an active member of SSS for a couple of years already or have at least one month contribution payment, you can already apply for an SSS ID.
Follow these steps to apply for an SSS ID:
1. Download the SSS E6-Form
Fill up completely the form, list your beneficiaries (if any), double check your spelling and sign the paper.
2. Submit the SSS E-6 Form in any SSS Office
Together with any of the following IDs:
- Driver’s License
- PRC License
- Seaman’s Book
If you don’t have the primary IDs mentioned above, you may submit two (2) of the following documents:
- Company ID
- School ID
- Postal ID
- Philhealth ID Card
- GSIS Card
- Tax Identification Card
- Voter’s Identification Card
- NSO Birth Certificate
- Marriage Contract
- NBI Clearance
- Police Clearance
- ID Card issued by Local Government Units
- ID Card issued by Professional Associations
3. Visit the nearest SSS branch office in your area
Be early as possible and get a number from a clerk in booth. If you arrive at 6am, you’ll finish by 10am.

*SSS offices are also open during Saturdays from 8am to 5pm
4. Wait for 30 days
You read it right, it usually takes 30 days before you receive your SSS ID at your doorsteps.
